Tocilizumab plus methotrexate is a combination therapy used primarily for the treatment of moderate to severe rheumatoid arthritis, especially in patients who have not responded adequately to conventional disease-modifying antirheumatic drugs (DMARDs). Tocilizumab is a humanized monoclonal antibody that targets both soluble and membrane-bound interleukin-6 receptor (IL-6 receptor), thereby inhibiting IL-6-mediated pro-inflammatory signaling. Methotrexate is a small molecule antimetabolite that inhibits dihydrofolate reductase, suppressing lymphocyte proliferation and modulating immune function. The combination provides superior clinical efficacy compared to monotherapy with either agent alone, more rapidly suppressing inflammation and preventing joint destruction in rheumatoid arthritis. Tocilizumab can be used as monotherapy or in combination with methotrexate; however, the addition of methotrexate may enhance clinical outcomes for some patients[1][3][5][8].
