A combination therapy of **tofogliflozin**, a selective sodium-glucose cotransporter 2 (SGLT2) inhibitor, and a **glp-1 analogue (glucagon-like peptide-1 receptor agonist)**, used for the treatment of **type 2 diabetes mellitus** in patients with inadequate glycemic control on monotherapy. Tofogliflozin lowers blood glucose by inhibiting renal glucose reabsorption, leading to glucosuria. GLP-1 analogues stimulate glucose-dependent insulin secretion, suppress inappropriate glucagon secretion, and slow gastric emptying. The complementary mechanisms address hyperglycemia, weight management, and blood pressure reduction, with reduced risk of hypoglycemia relative to sulfonylurea or insulin. Clinical studies support that combination therapy increases reduction in **HbA1c**, fasting plasma glucose, body weight, and systolic blood pressure compared to either monotherapy, while maintaining an acceptable safety profile. Adverse effects include increased risk of hypoglycemia, gastrointestinal symptoms (nausea, vomiting, diarrhoea), dehydration, constipation, thirst, pollakiuria, injection-site reactions, and genital infections[1][3][4].
