Tomaralimab (also known as NM-101 or NM-103) is a humanized IgG4 monoclonal antibody that targets Toll-like receptor 2 (TLR2). Developed by Neuramedy Co. Ltd., it is being investigated as a disease-modifying treatment for neurodegenerative synucleinopathies, specifically multiple system atrophy (MSA) and Parkinson's disease. The drug's mechanism involves blocking TLR2, which is implicated in the cell-to-cell transmission of alpha-synuclein aggregates and the activation of neuroinflammatory pathways in microglia. Originally developed by Opsona Therapeutics as OPN-305, the rights were acquired by Neuramedy in 2019. It is currently in Phase 1 clinical development to evaluate safety, tolerability, and pharmacokinetics in healthy volunteers and patients with MSA.
