topiramate + almotriptan

Topiramate + almotriptan is a combination of two pharmaceutical agents used in the management of migraine disorders. Topiramate is an anticonvulsant that also serves as a preventive treatment for migraines. Its mechanism involves blocking voltage-dependent sodium channels, enhancing GABA-A receptor activity, inhibiting glutamate activity at AMPA and kainate receptors, and weakly inhibiting carbonic anhydrase isozymes. These actions reduce neuronal excitability and help prevent seizures and migraine episodes[1][6]. Almotriptan is a selective serotonin (5-HT1B/1D) receptor agonist indicated for the acute treatment of migraine attacks with or without aura. It acts by causing vasoconstriction of cranial blood vessels and inhibiting neurogenic inflammation through activation of 5-HT1B/1D receptors on peripheral trigeminal nerve terminals[5]. The combination may be considered in clinical practice when both acute relief (almotriptan) and preventive therapy (topiramate) are needed for patients with frequent or severe migraines.
