topotecan + carboplatin

Combination chemotherapy regimen consisting of the small-molecule topoisomerase I inhibitor topotecan and the platinum-containing DNA-damaging agent carboplatin. Topotecan stabilizes the topoisomerase I–DNA cleavable complex, causing single-strand breaks that convert to double-strand damage during replication, while carboplatin forms intra- and interstrand DNA crosslinks via platinum adducts; together they produce schedule-dependent synergy and enhanced cytotoxicity in proliferating tumor cells. Investigated across multiple solid tumors including relapsed ovarian cancer and small cell lung cancer, with various schedules (e.g., topotecan days 1–5 with carboplatin on day 5) to balance efficacy and hematologic toxicity.
