topotecan + pazopanib

This is an investigational combination therapy consisting of topotecan and pazopanib, being developed by The Hospital for Sick Children (SickKids) in collaboration with the C17 Canadian Phase I Clinical Trial consortium. The treatment is designed for pediatric patients with relapsed or refractory solid tumors. It utilizes a low-dose metronomic (LDM) oral regimen of topotecan, a topoisomerase I inhibitor, combined with pazopanib, a multi-targeted receptor tyrosine kinase inhibitor. Pazopanib targets VEGF receptors (VEGFR-1, -2, and -3), PDGFR, and c-Kit to inhibit tumor angiogenesis. The combination aims to leverage the synergistic anti-angiogenic effects of metronomic chemotherapy and targeted VEGF pathway inhibition. A Phase I study (TOPAZ) established the recommended phase 2 dose and demonstrated that the combination is safe and tolerable in the pediatric population, with stable disease observed in a significant portion of participants.
