This combination therapy regimen consists of toripalimab, a humanized monoclonal antibody targeting the PD-1 receptor, and the mFOLFIRINOX chemotherapy regimen, which includes fluorouracil, leucovorin, oxaliplatin, and irinotecan. It is being investigated by Tianjin Medical University Cancer Institute and Hospital for the perioperative treatment of locally advanced resectable gastric adenocarcinoma and gastroesophageal junction adenocarcinoma. Toripalimab, developed by Shanghai Junshi Biosciences, acts as a checkpoint inhibitor by blocking the interaction between PD-1 on T cells and its ligands (PD-L1 and PD-L2), thereby enhancing anti-tumor immune responses. The mFOLFIRINOX component provides synergistic cytotoxic effects through multiple mechanisms, including DNA alkylation and topoisomerase inhibition. The regimen is currently being evaluated in a Phase II randomized trial (NCT04908566) to assess its safety and efficacy, specifically focusing on pathological complete response (pCR) rates in the perioperative setting.
