Toripalimab is a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, an immune checkpoint that normally limits T-cell activity. By blocking the interaction between PD-1 and its ligands (PD-L1 and PD-L2), toripalimab restores the ability of the immune system to recognize and eliminate cancer cells. In the clinical trial NCT05448183, sponsored by Xuhua Duan, toripalimab is being evaluated in combination with transarterial chemoembolization (TACE) for the treatment of extrahepatic cholangiocarcinoma. The TACE component of this regimen utilizes the cytotoxic agents gemcitabine, a nucleoside analog that inhibits DNA synthesis, and oxaliplatin, a platinum-based agent that forms DNA cross-links. This multi-modal approach aims to combine the localized tumor destruction and antigen release of TACE with the systemic immune-enhancing effects of PD-1 blockade to improve outcomes in patients with this aggressive bile duct cancer.
