Toripalimab + pemetrexed is a combination therapy consisting of toripalimab, a humanized monoclonal antibody targeting programmed death protein 1 (PD-1), and pemetrexed, an antifolate chemotherapy agent. Toripalimab acts as an immune checkpoint inhibitor by blocking the PD-1 pathway on T-cells, thereby enhancing the immune system’s ability to recognize and destroy cancer cells. Pemetrexed inhibits folate-dependent enzymes involved in nucleotide synthesis, leading to disruption of DNA replication and cell division in rapidly proliferating tumor cells. This combination has been approved for use as first-line treatment for patients with EGFR mutation-negative and ALK mutation-negative unresectable, locally advanced or metastatic non-squamous non-small cell lung cancer (NSCLC) in China[3][5]. The regimen is also being studied perioperatively for resectable NSCLC[2][4].
