Overview

Toripalimab + surufatinib + chemotherapy is an investigational combination regimen being evaluated for the second-line treatment of advanced RAS/BRAF mutant and microsatellite stable (MSS) metastatic colorectal cancer. Toripalimab is a humanized IgG4 monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, acting as an immune checkpoint inhibitor to restore T-cell activity. Surufatinib is a small-molecule inhibitor that targets vascular endothelial growth factor receptors (VEGFR-1, -2, and -3), fibroblast growth factor receptor 1 (FGFR1), and colony-stimulating factor 1 receptor (CSF-1R). The addition of surufatinib is intended to modulate the tumor microenvironment and enhance the efficacy of toripalimab and standard chemotherapy (such as oxaliplatin or irinotecan-based regimens). This specific combination was the focus of the Phase 2 APHRODITE study (NCT04653480) conducted at The First Affiliated Hospital, Zhejiang University School of Medicine, which aimed to address the limited efficacy of immunotherapy in MSS colorectal cancer patients.
