TBICy is a myeloablative conditioning regimen used prior to allogeneic hematopoietic stem cell transplantation (allo-HSCT), primarily for patients with acute lymphoblastic leukemia (ALL). It consists of total body irradiation (TBI) and high-dose cyclophosphamide. TBI serves as a potent anti-leukemic and immunosuppressive agent, capable of reaching sanctuary sites like the central nervous system and testes, while cyclophosphamide acts as a DNA alkylating agent to further eradicate malignant cells. The regimen is often compared to chemotherapy-only conditioning, such as busulfan plus cyclophosphamide (BuCy), to balance efficacy against toxicities like interstitial pneumonitis, cataracts, and secondary malignancies.
