Trabectedin + pegylated liposomal doxorubicin is a combination chemotherapy regimen primarily used in the treatment of platinum-sensitive recurrent ovarian cancer. Trabectedin is a marine-derived antineoplastic agent that binds to the minor groove of DNA and causes DNA backbone cleavage, leading to cell cycle arrest and apoptosis. It also modulates the tumor microenvironment by affecting tumor-associated macrophages and vascular endothelial cells[6]. Pegylated liposomal doxorubicin (PLD) is an anthracycline antibiotic encapsulated in a pegylated liposome for improved pharmacokinetics and reduced toxicity; it intercalates into DNA and inhibits topoisomerase II activity. The combination has demonstrated efficacy in improving progression-free survival (PFS) and overall survival (OS) in patients with BRCA-associated or platinum-sensitive recurrent ovarian cancers[2][3]. This regimen may be considered as an alternative to platinum-based therapies for patients who have progressed on PARP inhibitors[1].
