Tranexamic acid + standard of care refers to the use of tranexamic acid, an antifibrinolytic agent, administered in addition to the established or routine treatment protocols for a given condition—most commonly acute bleeding scenarios such as postpartum hemorrhage (PPH), trauma, or surgery. Tranexamic acid is a synthetic lysine analog that competitively inhibits plasminogen activation by binding to lysine sites on plasminogen molecules. This action prevents conversion of plasminogen to plasmin and stabilizes fibrin clots, thereby reducing bleeding[1][2][3][4]. The "standard of care" varies depending on indication but typically includes supportive measures such as fluid resuscitation, blood transfusion, uterotonics (for PPH), and surgical interventions as appropriate. The combination is used off-label in many settings beyond its FDA-approved indications (heavy menstrual bleeding and short-term prevention in hemophilia patients undergoing tooth extraction)[1][4]. Major trials like CRASH-2 and WOMAN have demonstrated survival benefits when tranexamic acid is added early to standard trauma or PPH management[2][4].
