Overview

Trastuzumab (Herceptin) is a humanized IgG1 monoclonal antibody that selectively binds with high affinity to the extracellular domain of the human epidermal growth factor receptor 2 (HER2). This specific therapeutic approach involves the continuation of trastuzumab in combination with a second-line chemotherapy agent (such as capecitabine, vinorelbine, or taxanes) in patients with HER2-positive metastatic breast cancer who have progressed on a first-line trastuzumab-containing regimen. The rationale for this 'treatment beyond progression' (TBP) strategy is that persistent HER2 signaling continues to drive tumor growth even after resistance to the initial chemotherapy develops. By maintaining HER2 blockade while switching the cytotoxic partner, clinicians aim to overcome resistance and extend clinical benefit. This paradigm was specifically evaluated in a Phase II trial sponsored by Hoffmann-La Roche.
