H + CT is a clinical shorthand notation for the combination of trastuzumab (H) and chemotherapy (CT), which is the standard-of-care first-line treatment for patients with HER2-positive advanced gastric or gastroesophageal junction adenocarcinoma. Trastuzumab is a humanized IgG1 monoclonal antibody that binds to the extracellular domain of the HER2 receptor, thereby inhibiting downstream signaling pathways and inducing antibody-dependent cellular cytotoxicity (ADCC). The chemotherapy component typically consists of a platinum agent (e.g., cisplatin) and a fluoropyrimidine (e.g., 5-fluorouracil or capecitabine), which exert broad cytotoxic effects by interfering with DNA replication and repair. This regimen was established as a standard following the results of the Phase 3 ToGA trial.
