Trastuzumab + CL264-ISAC is a preclinical immune-stimulating antibody conjugate (ISAC) developed as a research platform to evaluate the efficacy of Toll-like receptor (TLR) agonists in cancer immunotherapy. It consists of the HER2-targeting monoclonal antibody trastuzumab conjugated to CL264, a small molecule TLR7 agonist, via a non-cleavable linker. The conjugate is designed to target HER2-expressing tumor cells and simultaneously activate the innate immune system, specifically myeloid cells such as macrophages and dendritic cells, through TLR7 stimulation. This dual mechanism aims to convert immunologically "cold" tumors into "hot" tumors by promoting a localized pro-inflammatory microenvironment and priming adaptive anti-tumor immunity. In preclinical xenograft models, trastuzumab + CL264-ISAC demonstrated enhanced myeloid activation and anti-tumor activity compared to trastuzumab alone, although it was associated with some systemic cytokine secretion and transient weight loss in animal studies.
