Trastuzumab + natural killer cells is a combination immunotherapy approach that pairs the monoclonal antibody trastuzumab with adoptive transfer or ex vivo modification of natural killer (NK) cells. Trastuzumab is a humanized IgG1 monoclonal antibody targeting the extracellular domain of human epidermal growth factor receptor 2 (HER2), overexpressed in certain cancers such as breast and ovarian cancer. Its antitumor activity includes direct inhibition of HER2 signaling and induction of antibody-dependent cellular cytotoxicity (ADCC) via engagement with FcγRIIIa (CD16) on NK cells. In this combination strategy, either autologous or allogeneic NK cells are expanded and activated ex vivo—sometimes further armed by conjugation with trastuzumab using Antibody-Cell Conjugation (ACC) technology—and then infused into patients to enhance tumor cell killing through ADCC and direct cytotoxicity. This approach aims to overcome resistance mechanisms in HER2-positive cancers by boosting immune effector function at the tumor site. Notable investigational products include ACE1702, an off-the-shelf cryopreserved irradiated oNK cell line conjugated with trastuzumab for targeted killing of HER2-expressing tumors[1][6].
