Trastuzumab + paclitaxel + carboplatin + gemcitabine is a combination chemo-immunotherapy regimen investigated for the treatment of HER2-positive advanced or metastatic urothelial carcinoma. Trastuzumab is a humanized monoclonal antibody that selectively binds to the extracellular domain of the human epidermal growth factor receptor 2 (HER2), inhibiting downstream signaling pathways and inducing antibody-dependent cellular cytotoxicity (ADCC). In this specific regimen, evaluated by the University of Michigan Rogel Cancer Center, trastuzumab is combined with a triplet chemotherapy backbone: paclitaxel (a microtubule-stabilizing agent), carboplatin (a DNA-alkylating platinum compound), and gemcitabine (a nucleoside analog that inhibits DNA synthesis). This multi-targeted approach was designed to exploit HER2 overexpression in a subset of bladder cancer patients while providing broad cytotoxic coverage through multiple mechanisms of action.
