Trastuzumab + pertuzumab + taxane (THP) is a combination chemo-immunotherapy regimen used primarily in the neoadjuvant and first-line metastatic settings for HER2-positive breast cancer. The regimen consists of two HER2-targeted monoclonal antibodies, trastuzumab and pertuzumab, which provide dual HER2 blockade by binding to distinct epitopes (subdomain IV and subdomain II, respectively) of the HER2 receptor. This dual inhibition effectively prevents HER2 dimerization and downstream signaling while promoting antibody-dependent cellular cytotoxicity (ADCC). The taxane component (typically docetaxel, paclitaxel, or nab-paclitaxel) acts as a cytotoxic chemotherapy agent by binding to tubulin and stabilizing microtubules, leading to cell cycle arrest and apoptosis. The combination is currently being evaluated in trials such as neoDIRHP to determine if the number of chemotherapy cycles can be safely de-escalated in intermediate-risk patients.
