Trastuzumab + zenocutuzumab is an investigational combination of two monoclonal antibodies targeting the human epidermal growth factor receptor family, specifically HER2 and HER3. Trastuzumab is a recombinant humanized IgG1 monoclonal antibody that binds to the extracellular domain of HER2, inhibiting its signaling and mediating antibody-dependent cell-mediated cytotoxicity (ADCC). Zenocutuzumab is a bispecific IgG1 antibody engineered to bind both HER2 and HER3; it blocks neuregulin 1 (NRG1) from activating HER3, thereby disrupting oncogenic signaling through the PI3K-AKT and MAPK pathways. Zenocutuzumab also enhances ADCC via afucosylation. The combination has been studied in phase 2 trials in patients with heavily pretreated metastatic breast cancer overexpressing HER2, particularly those who have progressed on multiple lines of anti-HER2 therapy. The regimen aims to provide dual blockade of the HER2/HER3 axis, suppressing tumor proliferation and survival while engaging immune effector mechanisms[1][7][8].
