**Trastuzumab deruxtecan + nivolumab** is a combination of two drugs: trastuzumab deruxtecan (a HER2-targeted antibody-drug conjugate) and nivolumab (an anti–PD-1 monoclonal antibody immune checkpoint inhibitor). Trastuzumab deruxtecan consists of an anti-HER2 antibody, a tetrapeptide-based linker, and a topoisomerase I inhibitor payload, delivering cytotoxic effects selectively to HER2-expressing tumor cells[1]. Nivolumab blocks the PD-1 pathway, promoting T cell-mediated antitumor immune response[1]. The combination is intended to leverage both targeted cytotoxicity and immunotherapy, and has shown promising antitumor activity and manageable safety in HER2-expressing metastatic breast cancer and metastatic urothelial carcinoma[1][5].
