Overview

Trastuzumab rezetecan + retlirafusp alfa + CAPOX is an investigational combination regimen being evaluated for the perioperative treatment of resectable locally advanced gastric or gastroesophageal junction (GEJ) adenocarcinoma. The regimen combines three distinct therapeutic components: (1) trastuzumab rezetecan (SHR-A1811), a novel human epidermal growth factor receptor 2 (HER2)-targeted antibody-drug conjugate (ADC) carrying a topoisomerase I inhibitor payload; (2) retlirafusp alfa (SHR-1701), a bifunctional fusion protein designed to simultaneously block the programmed death-ligand 1 (PD-L1) and transforming growth factor-beta (TGF-beta) pathways; and (3) CAPOX, a standard chemotherapy doublet consisting of capecitabine and oxaliplatin. This combination is being evaluated in a randomized Phase II clinical trial sponsored by Tianjin Medical University Cancer Institute and Hospital to assess its safety, tolerability, and efficacy (measured by pathological complete response rate) in patients with resectable stage II-III gastric or GEJ adenocarcinoma.
