Trazodone + gabapentin is a combination of two pharmaceutical drugs, each with distinct mechanisms of action, investigated for synergistic effects in the management of neuropathic pain and potentially other conditions. Trazodone is an antidepressant classified as a serotonin receptor antagonist and reuptake inhibitor (SARI), primarily blocking post-synaptic serotonin 5-HT2A and 5-HT2C receptors while inhibiting presynaptic serotonin reuptake. Gabapentin is a GABA analogue that exerts its effect mainly by binding to the α2δ subunit of voltage-gated calcium channels, reducing presynaptic glutamate release. Preclinical studies and early clinical trials suggest that low-dose combinations may provide enhanced antinociceptive (pain-relieving) effects compared to either drug alone, likely due to their complementary mechanisms[2][4][6]. This combination has been studied particularly in the context of neuropathic pain such as painful diabetic neuropathy.
