AMG 386 + trastuzumab is a combination therapy evaluated in the neoadjuvant setting for high-risk, early-stage breast cancer. AMG 386 (trebananib) is a recombinant peptibody (peptide-Fc fusion protein) that acts as an angiogenesis inhibitor by binding to and neutralizing angiopoietin-1 and angiopoietin-2, thereby preventing their interaction with the Tie2 receptor. Trastuzumab is a humanized monoclonal antibody that targets the human epidermal growth factor receptor 2 (HER2). This combination was specifically studied in the I-SPY 2 (Investigation of Serial Studies to Predict Your Therapeutic Response with Imaging and Molecular Analysis 2) platform trial to assess whether adding an anti-angiogenic agent to HER2-targeted therapy and standard chemotherapy improves pathological complete response (pCR) rates in HER2-positive breast cancer patients. While the combination completed Phase 2 testing within the I-SPY 2 framework, it did not proceed to Phase 3 for this specific indication.
