Overview

The FLAT regimen is a combination chemotherapy conditioning strategy consisting of treosulfan, cytarabine (Ara-C), and fludarabine. It is primarily investigated as a pre-transplant conditioning therapy for elderly patients with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) undergoing autologous stem cell transplantation (HSCT). Treosulfan acts as a bifunctional alkylating agent that induces DNA cross-linking, while fludarabine and cytarabine are antimetabolites that inhibit DNA synthesis. Fludarabine also serves as a biochemical modulator to enhance the cytotoxicity of other agents. This specific regimen, often referred to as FLAT-Auto in clinical trials led by Fabio Ciceri, aims to provide an effective yet tolerable consolidation strategy for older patients who may not be candidates for standard high-dose conditioning or allogeneic transplantation.
