Triamcinolone + clobetasol propionate is a topical combination therapy comprising two synthetic corticosteroids of differing potencies. Clobetasol propionate is classified as a Class I 'super-potent' corticosteroid, while triamcinolone (typically administered as triamcinolone acetonide) is a Class II or III 'potent' to 'mid-strength' corticosteroid. This combination is utilized to provide intensive anti-inflammatory, antipruritic, and immunosuppressive activity for the management of severe, recalcitrant corticosteroid-responsive dermatoses, such as psoriasis, lichen planus, and resistant eczema. Both agents function by binding to the cytoplasmic glucocorticoid receptor (GR), which then translocates to the nucleus to modulate gene transcription. This results in the inhibition of pro-inflammatory cytokines and the induction of lipocortins, which subsequently inhibit phospholipase A2 and the release of arachidonic acid. While the dual-steroid approach aims to maximize local anti-inflammatory effects, it also carries an increased risk of localized adverse effects, including skin atrophy and striae.
