Triamcinolone + fibrin glue is an experimental combination therapy consisting of the synthetic corticosteroid triamcinolone and a biocompatible surgical adhesive, fibrin glue. Triamcinolone acts as a potent anti-inflammatory and immunosuppressive agent by binding to glucocorticoid receptors, thereby inhibiting multiple inflammatory cytokines and mediators. Fibrin glue (fibrin sealant) is composed of human-derived fibrinogen and thrombin; when mixed, these components form a stable clot that adheres to tissues, providing hemostasis and serving as a drug delivery matrix. The combination has been studied for local application during surgery—such as spraying onto the atrial epicardium—to reduce postoperative inflammation and arrhythmias by maintaining prolonged contact of triamcinolone with target tissues while minimizing systemic exposure. Preclinical studies in animal models have shown that this mixture can significantly reduce postoperative atrial tachyarrhythmias, decrease inflammatory cell infiltration at the site of application, promote tissue regeneration with less fibrosis, and potentially lower side effects compared to systemic steroid administration[1][2][3].
