Triamcinolone acetonide + polidocanol is an investigational combination therapy being evaluated for the intralesional treatment of keloids and hypertrophic scars. Triamcinolone acetonide is a synthetic glucocorticoid corticosteroid that suppresses inflammation and inhibits collagen synthesis by binding to glucocorticoid receptors. Polidocanol is a non-ionic surfactant that acts as a sclerosing agent and local anesthetic; it works by damaging the vascular endothelium within the scar tissue, leading to vessel occlusion and reduced lesion vascularity. This dual mechanism aims to achieve superior flattening and symptomatic relief compared to monotherapy. The combination is currently being studied in clinical trials sponsored by Sohag University.
