Overview

Triamterene + hydrochlorothiazide (also known as co-triamterzide) is an FDA-approved, fixed-dose combination small molecule drug sold under brand names such as Dyazide and Maxzide. It combines triamterene, a potassium-sparing diuretic, with hydrochlorothiazide, a thiazide diuretic. The combination is indicated for the treatment of hypertension and edema in patients where hypokalemia is a concern. Triamterene works by blocking epithelial sodium channels (ENaC) in the late distal tubule and collecting duct, which reduces sodium reabsorption and decreases potassium secretion. Hydrochlorothiazide inhibits the sodium-chloride symporter in the distal convoluted tubule, increasing the excretion of sodium, chloride, and water. Together, these mechanisms provide additive antihypertensive and diuretic effects while mitigating the potassium loss typically associated with thiazide monotherapy.
