This combination therapy consists of Triprilumab, a monoclonal antibody targeting the programmed death-1 (PD-1) immune checkpoint, administered alongside the chemotherapy doublet gemcitabine and cisplatin. Developed by Zhejiang University, the regimen is being investigated for the treatment of advanced or unresectable intrahepatic cholangiocarcinoma. The addition of an immune checkpoint inhibitor to the standard-of-care chemotherapy backbone aims to enhance anti-tumor immune responses and improve clinical outcomes in patients with this aggressive biliary tract cancer. Triprilumab works by blocking the interaction between PD-1 on T cells and its ligands, thereby restoring the immune system's ability to recognize and attack cancer cells, while gemcitabine and cisplatin provide direct cytotoxic effects by inhibiting DNA synthesis and inducing DNA cross-linking.
