A combination investigational **vaccine-based immunotherapy** consisting of a trivalent ganglioside vaccine (containing GD2 and GD3 gangliosides stabilized as lactones, each conjugated to keyhole limpet hemocyanin/KLH) with **OPT-821**, a purified saponin adjuvant. OPT-821 is a semisynthetic QS-21 equivalent derived from *Quillaja saponaria* and serves to enhance the immune response to the ganglioside antigens. Developed mainly for high-risk neuroblastoma in remission, the vaccine is administered subcutaneously, often in combination with oral β-glucan as an additional immune stimulant. Trials report immunogenicity and tolerability, with antibody responses generated against GD2 and GD3, and some documentation of effects on minimal residual disease[1][2].
