Overview

Trofosfamide + etoposide is an oral combination chemotherapy regimen used primarily in the treatment of various malignancies, including advanced soft tissue sarcoma and recurrent or progressive glioblastoma. Trofosfamide is an alkylating agent and a prodrug of ifosfamide that interferes with DNA replication by forming cross-links in DNA strands. Etoposide is a topoisomerase II inhibitor that induces DNA strand breaks by preventing the re-ligation of double-stranded breaks created during DNA replication. The combination has been shown to be well tolerated and effective as palliative therapy for adult patients with advanced soft tissue sarcoma, especially those who are heavily pretreated or have comorbidities limiting standard regimens[3]. In glioblastoma (both pediatric and adult), this regimen has demonstrated feasibility and safety, with some evidence suggesting potential survival benefits at recurrence[1][6]. Hematological toxicities are common adverse events.
