TT-816 + pembrolizumab is a combination therapy currently in clinical development for advanced solid tumors, including non-small cell lung cancer, ovarian cancer, and renal cell carcinoma. TT-816 is a first-in-class, orally available small molecule antagonist of the cannabinoid CB2 receptor, developed to modulate immune response by blocking the immunosuppressive effects mediated via the CB2 receptor on immune cells, thereby promoting T and NK cell infiltration and function within tumors. Pembrolizumab (Keytruda), developed by Merck, is a monoclonal antibody that binds to PD-1 (programmed cell death protein-1) on T cells, inhibiting its interaction with PD-L1/PD-L2 and thereby unleashing antitumor immune responses. The rationale for the combination is the potential for additive or synergistic enhancement of antitumor immunity by blocking both CB2 and PD-1 mediated immunosuppression. Early clinical trials (as of 2022/2023) are assessing safety, pharmacokinetics, and preliminary efficacy in patients with difficult-to-treat advanced solid tumors[1][2][3][5].
