TTFields + pembrolizumab is a combination regimen that includes Tumor Treating Fields (TTFields), a noninvasive regional therapy delivering low-intensity, alternating electric fields via surface electrodes (Optune device), and pembrolizumab, a programmed death-1 (PD-1) checkpoint inhibitor monoclonal antibody (Keytruda). TTFields disrupt mitosis by interfering with mitotic spindle formation and induce immunogenic cell death, activating type-1 interferon pathways and inflammasomes (notably STING and AIM2). Pembrolizumab blocks PD-1 on T cells, enhancing antitumor immune responses. Together, preclinical and early clinical data indicate synergistic efficacy, with TTFields amplifying immune activation and pembrolizumab enabling more sustained, effective antitumor immunity. This combination is under clinical investigation for newly diagnosed glioblastoma and advanced/metastatic NSCLC, improving survival outcomes and demonstrating acceptable tolerability[3][5][7][4][6].
