Overview

The combination of tucatinib, palbociclib, and letrozole is an all-oral regimen investigated for the treatment of hormone receptor–positive (HR+), human epidermal growth factor receptor 2–positive (HER2+) metastatic breast cancer. Tucatinib is a selective HER2 tyrosine kinase inhibitor; palbociclib is a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor; and letrozole is an aromatase inhibitor that suppresses estrogen production. This triplet targets three key pathways—HER2 signaling, cell cycle progression via CDK4/6, and estrogen-driven proliferation—to overcome resistance mechanisms in HR+/HER2+ breast cancer. The regimen has shown clinically meaningful efficacy and manageable safety in phase Ib/II clinical trials for patients previously treated with multiple HER2-targeted therapies[1][2][8].
