Tumor-infiltrating lymphocytes (TILs) developed by Karolinska University Hospital are an autologous adoptive cell therapy being investigated for the treatment of metastatic melanoma. The therapy involves harvesting naturally occurring T cells from a patient's resected tumor tissue, followed by their activation and large-scale expansion ex vivo. These expanded TILs are then re-infused into the patient after a non-myeloablative lymphodepleting conditioning regimen, typically consisting of cyclophosphamide and fludarabine. To support the survival and anti-tumor activity of the infused cells, systemic interleukin-2 (IL-2) is administered. In specific clinical protocols at Karolinska, this TIL therapy is also evaluated in combination with an autologous dendritic cell vaccine to further enhance the immune response against melanoma-specific antigens.
