Overview

The Two-Step Allogeneic Hematopoietic Stem Cell Transplantation protocol is a specialized cellular therapy platform developed at the Sidney Kimmel Cancer Center at Thomas Jefferson University. This approach is primarily designed for haploidentical (half-matched) donor transplants in patients with hematologic malignancies. The protocol separates the infusion of donor T cells from the hematopoietic stem cells to optimize the graft-versus-tumor effect while minimizing graft-versus-host disease (GVHD). In Step 1, patients receive a fixed dose of donor lymphocytes (DLI). This is followed by the administration of high-dose cyclophosphamide, which selectively depletes alloreactive T cells that have been activated by the recipient's antigens. In Step 2, CD34-selected hematopoietic stem cells are infused to achieve hematologic recovery. This method allows for rapid immune reconstitution and has been evaluated in various leukemias, lymphomas, and myelodysplastic syndromes using both myeloablative and reduced-intensity conditioning regimens.
