UB-421 + chidamide is an investigational combination therapy comprising two agents: UB-421, a monoclonal antibody that inhibits HIV entry by binding to the CD4 receptor on T cells, thus blocking viral attachment, and chidamide, a subtype-selective oral histone deacetylase (HDAC) inhibitor that acts as a latency-reversing agent by promoting HIV provirus transcription from the host genome. Chidamide targets class I HDAC1, HDAC2, HDAC3, and class IIb HDAC10, modulating epigenetic regulation to induce acetylation of chromatin histones. The combination is being evaluated in people living with HIV to reduce the size of the latent viral reservoir and potentially enable ART-free virological control. UB-421 is developed by United BioPharma and acts as an immunotherapy targeted at HIV; chidamide was developed by Chipscreen Biosciences and is used for its epigenetic modulation properties. This combination is in early-phase clinical trials for HIV cure-related strategies, specifically for reduction or control of HIV reservoirs during or post-ART interruption[1][3][5].
