UD TGFbi NK cells + temozolomide is an investigational combination immunotherapy regimen being evaluated for the treatment of stage IV melanoma, particularly cases with brain metastases. The regimen consists of allogeneic natural killer (NK) cells derived from universal donors (UD) that have been expanded ex-vivo and 'imprinted' to be insensitive to Transforming Growth Factor-beta (TGF-beta). TGF-beta is a potent immunosuppressive cytokine frequently found in the tumor microenvironment that typically inhibits NK cell function; by rendering these cells TGF-beta insensitive (TGFbi), their anti-tumor cytotoxic activity is preserved. Temozolomide, an oral alkylating agent, is administered as part of the protocol to serve as a lymphodepleting agent, creating a more favorable environment for the expansion and persistence of the infused NK cells, while also providing direct anti-neoplastic activity.
