Umbilical cord-derived Wharton's jelly mesenchymal stem cells (WJ-MSCs) are multipotent stem cells isolated from the mucoid connective tissue of the umbilical cord. Developed by the Shiraz University of Medical Sciences, these cells are being evaluated in a Phase III clinical trial for the prevention of heart failure in patients who have suffered an acute myocardial infarction (AMI) with impaired left ventricular function. The therapeutic rationale is based on the cells' ability to migrate to injured tissues and exert cardioprotective effects through paracrine signaling, immunomodulation, and the promotion of angiogenesis. By secreting various growth factors and cytokines, WJ-MSCs help reduce inflammation, limit adverse ventricular remodeling, and potentially stimulate endogenous repair of the infarcted myocardium. In clinical settings, they are typically administered via intracoronary infusion shortly after primary percutaneous coronary intervention.
