Overview

Unmanipulated UCB (umbilical cord blood) refers to a standard, non-modified unit of allogeneic hematopoietic stem and progenitor cells derived from donor cord blood. In clinical research conducted by the Masonic Cancer Center at the University of Minnesota (notably trial MT2008-15), it is utilized as part of a double-unit UCB transplant strategy for patients with high-risk hematological malignancies. In this protocol, one UCB unit remains unmanipulated to provide a baseline of hematopoietic cells, while a second, typically smaller unit is ex vivo primed with Complement 3a (C3a) fragment to enhance homing via the CXCR4–SDF-1 axis. Clinical data indicates that the unmanipulated unit often predominates in long-term engraftment due to higher doses of CD3+ T cells and total nucleated cells compared to the manipulated unit. This cellular therapy is intended to reconstitute the recipient's hematopoietic system and provide a graft-versus-tumor effect following nonmyeloablative conditioning.
