Overview

A three-drug combination of uproleselan, cytarabine, and idarubicin used as an investigational regimen for the treatment of acute myeloid leukemia (AML), particularly in newly diagnosed older adults and in relapsed/refractory AML. **Uproleselan** is a novel, specific E-selectin antagonist (GMI-1271) that blocks E-selectin on vascular endothelium, disrupting leukemia-microenvironment interactions, thereby sensitizing leukemia cells to chemotherapy and potentially reducing mucosal toxicity. **Cytarabine** and **idarubicin** are established chemotherapy agents: cytarabine is a nucleoside metabolic inhibitor inhibiting DNA synthesis, while idarubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II. This combination, investigated in phase 1/2 studies and ongoing later-phase trials, has shown high remission rates (CR/CRi), low induction mortality, and a favorable tolerability profile in AML patients, especially among the elderly and high-risk subgroups[5][7].
