Overview

VAC85135 + ipilimumab is a **combination immunotherapy regimen** investigated for the treatment of myeloproliferative neoplasms (MPN). VAC85135 is an investigational neoantigen vaccine designed to target tumor-associated mutations in calreticulin (CALR) and JAK2V617F by delivering viral vectors encoding relevant protein fragments, aiming to train the immune system to recognize and attack cancer cells displaying these neoantigens. Ipilimumab is a monoclonal antibody immune checkpoint inhibitor that targets CTLA-4 on T-cells, thereby enhancing anti-tumor immune responses. In the Phase 1 study (VAC85135MPN1001), this combination was assessed for safety, immunogenicity, and preliminary anti-tumor activity in adults with MPNs expressing mutant CALR or JAK2. The regimen included heterologous prime-boost vaccination with VAC85135 (intramuscular injection) alongside escalating and then fixed doses of intravenous ipilimumab. The study found that the combination was well tolerated but largely failed to induce strong neoantigen-specific immune responses or meaningful disease modification within the study period. Further development for this indication has been discontinued[1][3][5].
