valacyclovir + paracetamol + gabapentin is a combination therapeutic regimen utilized as standard medical therapy for the management of acute herpes zoster (shingles) and the prevention of postherpetic neuralgia. The regimen consists of three small molecule agents with distinct mechanisms: valacyclovir, an antiviral prodrug that inhibits viral DNA polymerase to suppress viral replication; paracetamol (acetaminophen), a non-opioid analgesic that provides pain relief likely through central prostaglandin synthesis inhibition; and gabapentin, an anticonvulsant that binds to the alpha-2-delta subunit of voltage-gated calcium channels to modulate the release of excitatory neurotransmitters involved in neuropathic pain. This specific combination was evaluated by Assiut University in a Phase 3 clinical trial as the control arm and as a baseline therapy alongside ultrasound-guided thoracic paravertebral blocks to assess improvements in pain control and the reduction of chronic neuralgia incidence.
