Overview
Valacyclovir + pregabalin + mecobalamin is a three-drug combination regimen used as a standard-of-care treatment for acute herpes zoster (shingles) and its associated pain. The regimen consists of: (1) **valacyclovir**, an antiviral prodrug of acyclovir that inhibits viral DNA polymerase to suppress Varicella-Zoster virus (VZV) replication; (2) **pregabalin**, a calcium channel alpha-2-delta ligand that acts as an analgesic and anticonvulsant to manage neuropathic pain; and (3) **mecobalamin** (methylcobalamin), a form of vitamin B12 that serves as a neurotrophic agent to support nerve repair and myelination. This combination is utilized in clinical research, such as the multicenter trial NCT07515885 led by Dexiong Han, as a comparator arm to evaluate the efficacy of interventions like electroacupuncture in reducing acute pain and the incidence of postherpetic neuralgia (PHN).
