A combination antibiotic therapy consisting of vancomycin and polymyxin B. This combination demonstrates synergistic activity against multi-drug resistant (MDR) bacteria, particularly carbapenem-resistant and polymyxin-resistant strains like *Acinetobacter baumannii* and *Klebsiella pneumoniae*. The combination allows vancomycin, which normally cannot penetrate the outer membrane of Gram-negative bacteria, to exert its antimicrobial effect against these pathogens when used with polymyxin B, which disrupts the bacterial outer membrane. While effective, this combination is associated with increased risk of nephrotoxicity compared to polymyxin B alone.
