Vancomycin + rifampin is a combination of two antibiotics used primarily for the treatment of serious infections caused by Gram-positive bacteria, especially those involving prosthetic devices or biofilm-associated infections. Vancomycin is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis by binding to D-Ala-D-Ala termini of cell wall precursor units, while rifampin is a rifamycin derivative that inhibits bacterial DNA-dependent RNA polymerase, thereby blocking RNA synthesis. The combination is particularly valued in treating prosthetic joint infections and endocarditis due to Staphylococcus aureus (including MRSA) and coagulase-negative staphylococci, as it can enhance efficacy compared to monotherapy and help prevent the emergence of resistance. However, use must be carefully monitored due to potential toxicities (nephrotoxicity with vancomycin; hepatotoxicity and drug interactions with rifampin) and the risk of rapid resistance development if either agent is used alone[1][2][5].
