This is a compounded topical formulation containing three active pharmaceutical ingredients: vancomycin, tobramycin, and itraconazole. Vancomycin is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis by binding to D-Ala-D-Ala termini of cell wall precursor units, primarily targeting Gram-positive bacteria. Tobramycin is an aminoglycoside antibiotic that binds the 30S ribosomal subunit, inhibiting protein synthesis and exhibiting bactericidal activity against many Gram-negative organisms. Itraconazole is a triazole antifungal agent that inhibits fungal cytochrome P450-dependent enzyme lanosterol 14α-demethylase, disrupting ergosterol synthesis and compromising fungal cell membrane integrity. This combination ointment is designed for the treatment of localized skin infections where both bacterial (including resistant strains) and fungal pathogens may be present or suspected—such as cutaneous fungal infections, bacterial skin infections, wound infections (including diabetic foot ulcers), and surgical site infections[1].
