**Vasopressin + methylprednisolone** is a combination of two drugs administered together during cardiopulmonary resuscitation for **in-hospital cardiac arrest**. Vasopressin is a nonapeptide hormone acting as a vasoconstrictor via agonism at the vasopressin V1 receptor, increasing systemic vascular resistance and thus arterial blood pressure. Methylprednisolone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressant properties, acting mainly through the glucocorticoid receptor to modulate gene transcription and reduce inflammation. Clinical trials have specifically studied this combination as adjunctive therapy to epinephrine in cardiac arrest. Evidence shows this drug combination significantly increases the probability of return of spontaneous circulation (ROSC) compared to placebo in adults with in-hospital cardiac arrest, although it does not improve long-term survival or neurologic outcomes. The mechanism is presumed to be vasoconstrictive support combined with mitigation of inflammatory injury during resuscitation[1][2][4][8].
