This combination refers to the co-administration of **vedolizumab** (an anti-integrin monoclonal antibody), **tacrolimus** (a calcineurin inhibitor), and **methotrexate** (an antimetabolite immunosuppressant). Each of these agents has an established use in the management of immune-mediated diseases: - **Vedolizumab** selectively binds α4β7 integrin on lymphocytes, blocking gut-specific lymphocyte trafficking, and is approved for moderate-to-severe inflammatory bowel disease (IBD) including ulcerative colitis (UC) and Crohn’s disease (CD). - **Tacrolimus** inhibits calcineurin and thereby blocks T-cell activation and cytokine production. - **Methotrexate** is a folate antimetabolite that inhibits DNA synthesis and possesses immunosuppressive actions. While there is clinical and preclinical evidence describing combination therapies of vedolizumab with tacrolimus[1][3][4], and vedolizumab with methotrexate[2], there is currently no widespread documentation supporting the routine clinical use of all three together in a single regimen—this triplet would be highly immunosuppressive and considered experimental, if used at all, likely for refractory IBD or severe autoimmune disease in select, closely monitored cases. The use, safety, and efficacy of this specific triple regimen is not established in guidelines or major studies, and should be considered investigational.
